Minimum Requirements for T1 (Transfer from Other UP Units) to UPLB-CAS

A. Accomplished Undergraduate Admission Application (UPLB Form No. 3)
B. Application letter addressed to the College Secretary

The letter should be signed by the student and his/her parent/guardian. The letter should also include the following:

      • Justification for transfer
      • Student’s contact number(s) and email address

C. True Copy of Grades (TCG) in all semesters enrolled

Note: All three documents should be submitted BOTH in printed and digital copies.

Printed copies:

    • Should be sent to CAS-OCS mailing address through courier from Monday to Friday (8:00 AM to 5:00 PM)
