Minimum Requirements for Shifting to CAS
A. Accomplished Application for Shifting Form (OUR Form 2021-6)
B. Application letter addressed to the College Secretary

The letter should be signed by the student and his/her parent/guardian, and noted by the adviser, department chair/unit director, and the College Secretary of the present college. For applicants within CAS, there is no need for the signature of the College Secretary. The letter should also include the following:
- Justification for shifting
- Student’s contact number(s) and email address
C. True Copy of Grades (TCG) in all semesters enrolled
Note: All three documents should be submitted in a single PDF file with the file name: Surname_Shiftee_DegreeProgramAppliedFor.pdf (e.g. DelaCruz_Shiftee_BSBIO.pdf).
